Does anyone know of any reason an employer would file a 1502 form, mark "OTHER" in section 2, with no explanation given below as required, when "F", First Report, should of been marked. It looks like they never sent a copy of the 801 form I filled out, which also should of been attached to the 1502 and sent to the WCD. Sounds fishy to me. Any response would be appreciated. Thanks! |

Take care If you're unsure of your injured worker rights or each time you think the insurance carrier violates the law, it's imperative that you call the Oregon Ombudsman for assistance and also so the violation can be documented.
